Angela Poe, RN

Angela Poe, RN, BSN is a compassionate and highly experienced registered nurse with over 20 years of service in both adult and pediatric home care. Her career has been defined by a deep commitment to client-centered care, strong clinical advocacy, and effective care coordination across a wide range of home health environments.

With a robust background in home care leadership, Angela has successfully led interdisciplinary teams, optimized service delivery, and championed high standards of quality of care and compliance. Her leadership experience spans intake coordination, case management, and client service operations—always with a focus on building trusting relationships with families and improving care outcomes.

Angela’s greatest strength is her ability to connect with families while delivering clinically sound, compassionate care. As a trusted clinician, she partners closely with clients and their loved ones to ensure care is personalized, transparent, and responsive to changing needs. Her deep clinical knowledge is matched by her natural empathy, allowing her to guide families through complex decisions with clarity and reassurance. Angela is known for building trust quickly, fostering open communication, and creating a sense of stability and confidence for those she serves.

Claudia Bastardo Armenta, RN, BSN

Claudia Bastardo Armenta, RN, BSN, is a compassionate and driven registered nurse who brings both clinical skill and genuine care to every family she supports. Fluent in English and Spanish, Claudia ensures that language is never a barrier for families reviewing their care options or feeling heard during a difficult time.

Claudia’s nursing career began in a blood cancer unit, where she learned early on that showing up for patients means more than managing their medical needs—it means being present during some of the hardest moments families face. She administered chemotherapy, managed transfusions and central lines, and provided education and emotional support to patients navigating complex diagnoses. That experience taught her how to stay calm under pressure, communicate with clarity, and offer reassurance when families need it most.

As a Client Care Supervisor, Claudia partners with families to ensure their care plans reflect what matters most to them. She approaches every conversation with empathy and respect, understanding that choosing care for a loved one is deeply personal. Her ability to listen, explain options clearly in both languages, and guide families through uncertainty makes her a trusted advocate for the people she serves.

Laura Serrano Rivera, RN, BSN

Laura Serrano Rivera, RN, BSN, is a thoughtful and empathetic registered nurse. She has almost 10 years of nursing experience, including hospital, private duty nursing home care, and clinical case management. Her background has shaped her deep understanding of patient-first support. Fluent in both English and Spanish, Laura offers language access and cultural understanding to Colorado families navigating complex care needs.

Laura’s nursing career began with six years in a hospital Progressive Care Unit. She also spent two years simultaneously in pediatric private duty nursing, where she provided one-on-one in-home care. Next, she spent 18 months working in home care clinical case management. Her approach to care stems from the recognition that families are managing unknowns, logistics, and support all at once. Her clinical background also includes a year in an intensive care unit, where she gained extensive experience managing critically ill patients and their complex medical needs.

After working as a clinical case manager, Laura knows that successful home care isn’t just about coordinating providers; it’s about finding the right support plan. She can efficiently navigate healthcare systems and is always a strong advocate for care recipients and their families.

Laura approaches every family as a partner. Her calm presence and clinical rigor create space for families to ask hard questions, make informed decisions, and feel truly supported. Her ability to bridge clinical expertise and hands-on home care with genuine empathy allows her to guide families through difficult transitions with clarity and reassurance.
